Enclosed in this section are a number of frequently used forms that Buffalo Bison players and their families need in order to properly register for the season.

A Player Contract is required of every skater in the program:

Buffalo Bison MOHL Player Contract for 2008-2009  

Code of Conduct was designed for every Buffalo Bison player, parent and Head Coach to read and sign before the season gets underway. We also submit this form when we enter the season-ending New York State Championships. The form parallels USA Hockey's format, however, we have modified it slightly to reflect our program values.

Medical Information Form Waiver of Liability - USA Hockey was designed by USA Hockey. We capture this information in case your child is injured and the coach needs to know certain information about your child before medical treatment can be provided.

Consent to Treat Form was also designed by USA Hockey. It is intended to give a coach or manager permission to obtain medical attention for any child who may be injured during a contest when parents are not present. 

Player Release Forms, designed by NYSAHA, are for players who wish to tryout for another organization and are in good standing with Buffalo Bison Hockey.  Once a release form is signed a delivered, a player is no longer a part of Buffalo Bison Hockey until he or she signs another contract.
